The table is constructed so that the elements contained in the columns are always paged with a left side alignment.
Cells containg the chapter titles have a colspan="2" to aligns the chapter titles between each other. Furthermore these cells contain bookmarks correlated with its anchor in the "Contents Table".
The chapter texts are alligned with the title page
Mandatory:
1) specify the measurement in pixel of the width of the Table;
2) assign the measurements of the columns (td), but only in the cells in grey:
3) insert inside the first row (coloured in grey) the images blank_02.gif, these two images are necessary to leave a space of 90 pixel (and 35 pixel) from the margin left of the Browser window and the texts inside the table.
4) use 2 <br> , above chapter title, to leave two empty lines of text as space between the previous argument and the chapter title of the new one. Use 1 <br> above the text to leave an empty line as space between the chapter title and the text itself.
